RWJF Social Determinants Learning Report

Social determinants, as readers of this Editorial know, are those everyday factors like housing, food, and employment that influence our health and health outcomes. The Robert Wood Johnson Foundation (RWJF) has been a leading source of information on the high cost we pay as a society when we leave social determinants unmanaged and unaddressed. To that end, RWJF has collected enormous amounts of data and consistently presented research showing why this is one of the most important issues facing American health care.

Smokers have a harder time getting jobs, and when they do get jobs, they earn less than nonsmokers, a Stanford University study found. Those findings, which recently made headlines in NY Daily News, Philly.com and other news outlets, are especially sobering given smoking's well-known, detrimental health effects.

Healthify Raises $2.5 Million to Improve Community Health

Healthify, whose mission is to change the way healthcare addresses community health, today announced the close of a $2.5 million seed round led by a syndicate of investors including Primary Venture Partners (PVP), Milestone Venture Partners (MVP), Acumen, Kapor Capital, and Blue Cross and Blue Shield of Kansas. Healthify works with managed care organizations and healthcare providers to better connect patients with community services to resolve unmet social needs. The company already provides services in 24 states and raised the money to expand its coordination feature set into a nationwide network of insurers, healthcare providers, and community agencies.

The Economic Cost of Domestic Violence

According to a new report by the McKinsey Global Institute, more than 39 million women — or about one-third of the country's adult female population based on U.S. census figures — experienced physical violence by an intimate partner, from slapping to beating.

Accountable Health Communities: Track 3 Alignment

Next week — May 18 — is the deadline for bridge organizations to submit their applications to CMS for the Accountable Health Communities (AHC) model. For the past few weeks, Healthify has been reviewing how software can support the goals of Track 1 Awareness and Track 2 Assistance. Today we review Track 3 Alignment — the largest and most complex of the three interventions proposed by CMS. Track 3 is a $4.5 million grant that will be awarded to 20 bridge organizations to answer the following question: Will a combination of community service navigation and partner alignment at the community level improve total health care costs and inpatient and outpatient health care utilization?

Accountable Health Communities: Track 2 Assistance

With the grant deadline for the CMS Accountable Health Communities (AHC) model just a couple of weeks away, Healthify is continuing its review of all three tracks of the AHC model. Today we take a look at Track 2 Assistance — a $2.5 million dollar grant over five years — that will allow 12 bridge organizations to answer the following question raised by CMS: “Will providing community service navigation to assist high-risk beneficiaries with accessing community services to address identified health-related social needs impact their total health care costs, inpatient and outpatient health care utilization, and health and quality of care?”
