Healthify Joins WellSky® to Address SDoH and Advance Social Care

If you haven’t already heard the exciting news, Healthify is now part of WellSky.

WellSky is leading the movement for intelligent, coordinated care worldwide. The technology company provides software, analytics, and services to connect organizations from across the care continuum to better serve their communities.

This strategic acquisition, announced on July 29th, will help Healthify fulfill our longstanding mission of building a world where no one's health is hindered by their need. Working with WellSky and its strategic partners, we aim to align payers, providers, and community organizations across the country to better address socioeconomic needs.

Why this matters

As healthcare continues the shift to value-based models of care, payers and providers are investing in capabilities that enable stakeholders to drive better outcomes, improve patient satisfaction, and increase savings.

WellSky enables value-based care at scale by providing seamless coordination between providers, community-based organizations, and payers.​ With the addition of Healthify, WellSky clients will have the ability to address social determinants of health more effectively across the care continuum to improve outcomes and remove social barriers to health.​

 

Health plans, hospitals, clinics, and providers will be able to better identify social needs, locate the most appropriate services, and coordinate care with WellSky’s network of more than 11,000 acute, post-acute, and community care providers, including 300+ Area Agencies on Aging (AAAs), 250 Continuums of Care (CoCs), and 100 I&R/A providers.
