Healthify Partners With Algorex to Expand SDoH Predictive Analytics



The combined offering leverages population health data, machine learning, and advanced analytics to inform interventions that address SDoH.

Healthify is partnering with healthcare data science company Algorex Health Technologies to offer social determinants of health (SDoH) predictive analytics. The enhanced tool will help payers and providers better understand and address the social needs of members and patients, which can impact overall health outcomes.

The Boston-based Algorex provides social determinant data analytics to support value-based care initiatives. Algorex also works to demonstrate the return on investment that SDoH interventions have for organizations and their patients.

The Algorex partnership boosts Healthify’s offering to build and manage accountable networks of community-based organizations (CBOs) and implement the technology to coordinate care across network partners. The integrated solution leverages population health data, machine learning, and advanced analytics to inform targeted inventions that address social inequities.

“Social determinants of health – such as access to where a person lives, the type of job they hold and their level of education – have a dramatic impact on health and wellbeing. Recently, the COVID-19 pandemic exposed how inequalities in access to food, housing, employment and other resources can put disadvantaged communities at greater risk,” said Manik Bhat, Founder and Chief Executive Officer of Healthify. 

“Together with Algorex we’re applying community-level data science and predictive analytics to help health plans and providers proactively identify at-risk populations and address social needs to improve health outcomes.”

Predictive analytics and community data can inform SDoH interventions 

The new offering integrates Algorex’s machine learning capabilities and advanced analytics into Healthify’s SDoH Platform to help healthcare organizations identify social needs at a granular level. Based on that data, Healthify’s platform identifies community-based organizations that can provide support for people in need, including food delivery, childcare, medical transportation, and utility support. 

“When building a robust SDoH program, healthcare organizations need to determine both the specific needs of their communities and what interventions would be most beneficial for them,” said Jacob Luria, Managing Partner at Algorex Health Technologies. 

“Predictive analytics is a critical first step in uncovering the non-clinical needs of patients and identifying social risk factors. We’re proud to partner with Healthify to apply our advanced analytics capabilities to equip healthcare payers, providers and community-based organizations with the specific, in-depth data they need to proactively address social needs through targeted interventions.”

Data-driven, robust interventions at the local level

The announcement formalizes and expands a previous collaboration to identify SDoH interventions for Excellus BlueCross BlueShield members.

“The key to long-term sustainability is the evaluation of impactable cost,” Luria said on a recent Healthify-hosted webinar to inform payers on how to use data to build scalable SDoH intervention strategies.

To understand the true scope of a community’s social needs, Luria advocates for community-level data sets to build digital profiles. Then, a payer or provider can identify and roll member profiles into specific cohorts. Using these cohort groups, organizations can plot SDoH programs that result in better outcomes and healthier populations. 

For example, Algorex identified and targeted more than 4,700 plan members out of 200,000 enrolled members for a BlueCross BlueShield plan building out an SDoH program using data analytics.

“It’s one thing to say ‘x is really important,’ but it's another to say with a level of confidence that ‘there are 3,600 individuals that have this need,’” Luria noted on the webinar.

Healthify expands its partnership roster

The Algorex deal adds to Healthify’s partnership portfolio. Recently, Healthify announced a partnership with Ride Health to offer transportation to medical and non-medical services to populations in need. In 2018, Healthify partnered with EHR data integration company Redox to use clinical data to inform a social needs care plan.
