Did you know, in New York State:
- Over 30% of births do not have early (1st trimester) prenatal care?
- 14% of births are to women aged 25 years and older without a high school education?
Last week, the Maternal and Infant Community Health Collaborative Team at Wyckoff Hospital joined Healthify’s Community Care Network! They are now able to access Healthify’s database of nearly 4,000 resources in New York City (and nearly 200,000 nationwide) in order to help their clients find what they need. The number 1 need on their list now? Baby strollers for new moms.
This team is 1 of 23 teams in New York State helping to improve maternal and infant health outcomes for high-need, low income women and their families. The team is tasked with finding and engaging high-need women and their families, assessing their needs, and then making referrals to services that can address these needs.
Wyckoff’s MICHC team helps to ensure new moms stay healthy before, during and after pregnancy. Welcome, MICHC Team!
