Social determinants, as readers of this Editorial know, are those everyday factors like housing, food, and employment that influence our health and health outcomes. The Robert Wood Johnson Foundation (RWJF) has been a leading source of information on the high cost we pay as a society when we leave social determinants unmanaged and unaddressed. To that end, RWJF has collected enormous amounts of data and consistently presented research showing why this is one of the most important issues facing American health care.
A Social Determinants Learning Report
The challenge RWJF is now tackling is what to do with all this data and how to use it in overcoming barriers that impede collaboration between health care organizations and social services. RWJF takes the forefront once again with its recently released "Using Social Determinants of Health Data to Improve Health Care and Health: A Learning Report" to discuss the challenges and opportunities that lie ahead. RWJF reached out to leading researchers, providers, and technology companies to compile this report. Healthify was proud to contribute to this report, and we invite you to read RWJF's findings in their report.
