We set out to answer those questions during the inaugural Gen-H Innovation Series last month. In collaboration with Healthify, the day-long session prompted conversations about: 

  1. Eliminating barriers between the different stakeholders in the community to achieve collective impact
  2. The needs of various stakeholders in the community to participate in collective impact initiatives
  3. And maximizing capacity and sustainability through partnerships with funders

Healthify Raises $16 Million to Integrate Health and Social Services

The latest funding marks a pivotal point in the movement toward value-based care 

Healthify, the leading company integrating the social determinants of health (SDoH) in the era of value-based healthcare, has raised $16 million in a Series B financing round led by SV Health Investors and existing investors like BlueCross BlueShield Venture Partners (BCBSVP). The latest round of funding provides strategic capital to fuel the company’s rapid growth as it continues to bring healthcare and social service organizations together in accountable networks. To date, Healthify has raised over $28 million through investments, grants, and awards.

The Partnership Will Address SDoH Across 97,000 Patients

On June 18th, Healthify and Landmark Health announced a groundbreaking collaborative partnership to address the social needs of 97,000 patients across 13 states—California, Kansas, Kentucky, Louisiana, Massachusetts, Mississippi, Missouri, New York, North Carolina, Ohio, Oregon, Pennsylvania and Washington. The partnership between Healthify and Landmark signifies a notable milestone in Landmark’s mission to deliver comprehensive care to complex, chronic patient populations wherever and whenever they need it.

The Role of Community-Based Organizations in Supporting Value-Based Care

In the transition to value-based care, providers today are being asked to do something they are not use to—caring for patients outside of the clinical setting. With incentives aligned on reducing utilization and decreasing re admissions, providers must now focus on prevention and wellness, in addition to delivering quality acute care.

This task, however, of managing population health is easier said than done. Given the many social circumstances that impact an individual’s care, it is often difficult to ensure positive outcomes after discharge.  Without active monitoring, providers can’t always guarantee patients follow the recommend care plan. And they can’t always verify that patients are being discharged to a place with suitable conditions for recovery.
